Liability Release

I agree to pay all Fees due to Mendocino Ballet. I understand fees are due the first class of each Session/month and that NO REFUNDS OR ADJUSTMENTS are made for any reason, unless class is cancelled by Mendocino Ballet. I understand and agree that any make-up classes are my responsibility and must be made within 30 days of the class missed, and as determined by the Instructor.

I recognize and understand the risks of physical injury inherent to dance and dance training and am willing to assume those risks for myself/my child(ren). I agree that I will not hold Mendocino Ballet (including any faculty, assistant, board member, or employee) liable for injuries sustained or illness contracted by me/my family while in attendance at Mendocino Ballet. I understand that it is my responsibility to safeguard my personal property and will not hold Mendocino Ballet responsible to loss or damage of personal property while in attendance at Mendocino Ballet. I also understand that the teaching of dance requires a certain amount of physical contact between teacher and student and that such physical contact is inherent in the teaching process and not meant to be inappropriate. Email addresses listed above will only be used by Mendocino Ballet for billing information, newsletters, and notifications of upcoming performances and/or events. They will NOT be given to any outside individuals, businesses, or organizations.

I further agree to allow the use of photographs and videos of my self/my child(ren) to be used for publicity purposes by Mendocino Ballet, and agree that such items are the sole property of the Mendocino Ballet.

I acknowledge that I have read, understand and agree to these policies by submitting payment for this class (applies to any type of payment: Registration Only, Class Only, and Class + Registration payments).
